Click on Submit Request under Online Services on the right side of the page.
Register to become an Online Services user or login.
Choose your request type by using the scroll down menu under Request Type.
Enter the property address.
You may describe your observations or explain the action needed in the Description box.
Complete the required contact information.
You will be given an Incident Number.
Check on the status of your request under Request Search where you can search by Incident Number, Incident Type or Address.
